Randy Alexander, Chairman of the Board

Randy Alexander is a leader in the ophthalmic industry. He has consistently created value, delivered growth and implemented new technology, including successfully launching four start-up companies.

He is the Chairman of ReVision Optics, Inc. and a seasoned senior executive and leader in the ophthalmic industry. Most recently as CEO of IntraLase Inc., Mr. Alexander grew the business from a start-up to over $18 million in revenue in a little over four years.

During this period, Mr. Alexander was instrumental in raising $75 million in capital for IntraLase. Prior to IntraLase, Mr. Alexander was Senior Vice President of Chiron Vision, where he established Chiron Vision as a leader in the refractive surgery industry. At Chiron Vision, he created a $50 million refractive product line by identifying the LASIK surgical procedure, as well as acquiring patents and instrumentation.

During his 28-year career in ophthalmology, Mr. Alexander has also served as CEO of Pharmacia Intermedics and Senior Vice President, Sales & Marketing of CILCO. Mr. Alexander holds bachelor’s and master’s degrees in Biological Sciences from Marshall University.



John T. Kilcoyne, Chief Executive Officer and President

John T. Kilcoyne brings more than 29 years of experience in the medical device market to RVO. He has a demonstrated record in understanding the needs of both patients and physicians combined with an ability to move companies from development into the commercial market.

Most recently he served as Chairman and CEO of Micrus Endovascular Corporation, until its sale to Johnson & Johnson in September 2010. Mr. Kilcoyne also previously served as President and Chief Executive Officer of Solace Therapeutics, a medical device company from 2002 to 2004. Prior to Solace, he served as the President and Chief Executive Officer of Endonetics, Inc. from 1997 until its acquisition by Medtronic in December 2001.

Mr. Kilcoyne received his B.S. from Cornell University.



Gilbert H. Kliman, MD

Gil Kliman, MD, is a Managing Director of InterWest Partners, a leading diversified venture capital firm. At InterWest, he focuses on life sciences investing with a special interest in medical devices and ophthalmology. He is a lead investor and board member for a broad portfolio of InterWest life sciences companies, including Epocrates (EPOC), Glaukos, On Demand Therapeutics, Restoration Robotics, and ReVision Optics. He was one of the early lead investors in IntraLase (ILSE), an ophthalmic laser company that completed a highly successful IPO in 2004 and was acquired by AMO in 2007 for $808 million, as well as LenSx, the first femtosecond laser cataract surgery company, which was acquired by Alcon in 2010 for a total transaction value of $744 million.

Prior to InterWest Partners, Dr. Kliman was a healthcare investor at TA Associates and at Norwest Venture Capital. As a former practicing ophthalmologist he was the West Coast Director of LCA Vision, and Assistant Professor of Ophthalmology at Tufts New England Eye Center. Dr. Kliman did a retina fellowship at Massachusetts Eye and Ear Infirmary as a Heed Foundation Fellow where he conducted and published the first studies of photodynamic therapy for macular degeneration. He completed his ophthalmology residency at Wills Eye Hospital, and holds an MD from the University of Pennsylvania, an MBA from Stanford, and a BA from Harvard. He also serves as a National Trustee for the Foundation Fighting Blindness.



Wende Hutton

Seasoned healthcare investor Wende Hutton is passionate about identifying, investing in and building companies that are revolutionizing the practice of medicine. She joined Canaan in 2004, bringing diverse expertise from across the entire medical spectrum. Over the last 15 years, Wende has facilitated the market entrance of over 11 novel and lifesaving medical devices, new drugs and diagnostics. Her track record is a testament to her leadership and includes the initial public offerings of five companies, of which two were later acquired, and the acquisition of four more by medical companies including the recent acquisition of BiPar Sciences, a leader in the development of a potential new class of tumor-selective drugs designed to meet the unmet needs of cancer patients. Wende began her venture career at Mayfield Fund in 1993, where she was a General Partner in the life sciences group until 2001. During her tenure at Mayfield, Wende incubated Heartstream Inc. (HPQ) and Northstar Neuroscience Inc. (NSTR), working closely with those founding teams. Most recently, she was a General Partner at Spring Ridge Ventures, a healthcare-focused seed fund. Earlier in her career, Wende held senior operational management positions at GenPharm International and Nellcor in business development and marketing.

Wende sits on the boards of Altheos, Inc., Calibra Medical, ReVision Optics, Inc., Theraclone Sciences and Transcend Medical, Inc. Her previous board seats include Alsius (ALUS) , Apieron, Inc. (Acquired by Aerocrine), BiPar Sciences (acquired by Sanofi-aventis), Heartstream, Inc. (acquired by Hewlett Packard), Micro Therapeutics, Inc. (acquired by ev3, Inc.), Northstar Neuroscience (NSTR), Orquest, Inc. (acquired by Johnson & Johnson), and SenoRx, Inc. (NASDAQ: SENO).

Wende holds an AB in human biology from Stanford University and an MBA from Harvard Business School, where she was a Baker Scholar. She is an active member of several community service groups, including serving as task-force leader on obesity for the Santa Clara and Mid-Peninsula YMCAs. Beyond her community service involvements, Wende’s first love is a field biology hike or lecture.



Frank M. Fischer

Frank M. Fischer has more than 30 years of senior management experience in the medical device industry. He has served on the NeuroPace Board of Directors since 1998 and joined the company as its Chief Executive Officer in January 2000.

Prior to joining NeuroPace, Mr. Fischer was President and Chief Executive Officer of Heartport, Inc., a cardiac surgery company, from May 1998 until September 1999 and served on Heartport’s Board of Directors. Mr. Fischer was President, Chief Executive Officer and a director of Ventritex, Inc., a company that pioneered implantable cardiac defibrillators, from 1987 until the sale of the company to St. Jude Medical, Inc. in early 1997.

Before joining Ventritex, he spent ten years in various management positions at Cordis Corporation in the cardiac and neurosurgical device areas, serving most recently as President of the Implantable Products Division. Prior to that, he spent seven years with General Electric Company in a variety of operating positions.

Mr. Fischer holds an MS in Management from Rensselaer Polytechnic Institute. Currently, he serves on the boards of several privately held medical device companies.



Brian H. Dovey

Mr. Dovey has been a Partner of Domain since 1988. Since joining Domain, he has served on the Board of Directors of over 35 private and public companies and has been Chairman of five. He is currently on the board of two public companies, Orexigen Therapeutics and REVA Medical.

Prior to Domain, Mr. Dovey spent six years at Rorer Group (now part of sanofi-aventis). As President of Rorer from 1986 to 1988, he was the primary architect of this company’s strategic shift to pharmaceuticals that resulted in a doubling of annual sales to approximately $1 billion. Previously, Mr. Dovey was President of Survival Technology, a start-up medical products company whose sales growth placed it in the top ten of the Inc 100. He has also held management positions with Howmedica, Howmet Corporation, and New York Telephone. Mr. Dovey has served as both President and Chairman of the National Venture Capital Association. He was former Chair and currently serves on the Board of Directors of the Wistar Institute, a leader in preclinical bio-medical research in the non-profit sector.

Mr. Dovey serves on the Board of Directors and is also Chairman at the Center for Venture Education (Kauffman Fellows Program). He is a Trustee Emeritus of Germantown Academy, and is a former Trustee of the University of Pennsylvania School of Nursing and the Burnham Institute for Medical Research. He was also a former board member of the industry associations representing the medical device industry as well as the association representing consumer pharmaceuticals.

Mr. Dovey received his BA from Colgate University and an MBA from Harvard Business School.



Jay T. Holmes

Jay T. Holmes retired as Executive Vice President and Chief Administrative Officer of Bausch & Lomb where he was also a member of the Board of Directors. Since retirement, he has been involved with a number of ophthalmic related companies including board of director service on the boards of Visx, Intralase, TLC Vision, and Occulogix. Other board of director service has included Rochester Energy Group and Blue Cross/Blue Shield.

Mr. Holmes is a Florida Supreme Court certified mediator which enables him to conduct a mediation practice.

He is a graduate of the University of Alaska and the University of Wisconsin Law School.



Jee Shin

Jee joined ProQuest in 2001 as an Associate and was promoted to Principal in 2004.

Prior to joining ProQuest, Jee served as the Director of Marketing and Business Development at Situs Corporation, a venture-backed developer of drug delivery systems. Previously, she served as a Marketing Associate at Guidant and as a Surgical Specialist at Johnson & Johnson. Prior to Johnson & Johnson, Jee worked at Boots Pharmaceuticals as a hospital sales specialist.

Jee currently serves on the Board of Directors of ReVision Optics and Zosano Pharma and represents ProQuest as an observer on the Board of Directors of Immune Design Corporation and Tragara Pharmaceuticals.

Jee was a board observer for Cadence Pharmaceuticals (Nasdaq: CADX), Concentric Medical (acquired by Stryker Corp., NYSE: SYK), Excaliard Pharmaceuticals (acquired by Pfizer, NYSE: PFE), Lead Therapeutics (acquired by BioMarin, Nasdaq: BMRN) and Optimer Pharmaceuticals (Nasdaq: OPTR). In addition, she worked closely with ProQuest portfolio companies Avanir (Nasdaq: AVNR) and Pharmion (acquired by Celgene, Nasdaq: CELG), among others.



Stephen Slade, MD, FACS

Dr. Slade is an active researcher and the U.S. medical monitor for several new technologies, and has been a consultant on many FDA clinical trials. In addition to his position at Slade & Baker Vision Center of Houston, Tex., Dr. Slade is Chief Medical Editor of Cataract and Refractive Surgery Today. In 1991, Dr. Slade and Stephen Brint, M.D., performed the first LASIK procedure in the U.S. and later performed the first Custom LASIK ablation-based on topography in the U.S. He also was the first to perform and has the longest experience with all-laser LASIK, laser cataract surgery and the accommodating IOL for presbyopia, the crystalens.

Dr. Slade was elected president of the American-European Congress of Ophthalmic Surgery in January 2011 and is a Fellow of the American Academy of Ophthalmology and the American College of Surgeons. He was named a Leading Ophthalmologist in America by Becker’s ASC Review. He is an active teacher of surgical techniques and has lectured and certified more than 8,000 physicians on LASIK and lamellar refractive surgery. Dr. Slade has received numerous awards including 11 named lectures, Refractive Surgeon of the Year, two China Service Medals, the Summit Technologies Pioneer of Refractive Surgery Award, the Casebeer Award and the Lans Lectureship. In 2007 he received the Barraquer Award, the highest award from the largest society of refractive surgeons. Dr. Slade was selected by his peers for both “Best Doctors” and “Best Doctors in America,” and has received the Honor Award of the American Academy of Ophthalmology.
